Your first visit to Stone Oak Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.

There's also background information about our committed staff.  You can also save some time by  completing the patient forms in advance of your appointment.

Kindly give 24 hour notice for all cancellations to avoid a charge.

Patient Forms.  Since the new healthcare law is taking place, we have implemented electonic medical records.

Please help  us provide a more efficient visit by filling out the forms prior to your visit.  THANK YOU!!!  Once you obtain your patient portal user name and password from our office you can log in.  Please click on the link below.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this Web site. Also, take some time to review our staff page and familiarize yourself with the doctor and her staff.  We look forward to your first visit.

Please be aware that COPAYS AND DEDUCTIBLES are payable at time of visit.  Please know that we do not know all the details of everyone's insurance. If you have questions, please call your insurance provider. 

PRIOR TO YOUR APPOINTMENT:

* If you have been seen by another dermatologist please have them send records.  Medical record release form is available under Patient Forms. 

* Come in without make-up or remove it prior to the appointment.

* Bring all your medications with you.

* Be prepared for a full body exam

* Minors must be accompanied by parent or legal guardian for first visit.  Minor consent forms may be signed to allow for subsequent visits.
